I purchased this beautiful stained glass angel pattern from Anypattern.com a number of years ago - the site has since been decommissioned.  The artist was recorded as Sonya Fetch out of Port Angeles WA and the pattern was identified simply as "Stained Glass Angel".  I haven't been able to locate any newer information on the artist or her work...
